AUTHORIZATION AND RELEASE

Should my application be accepted I agree to follow the policies of Oak Pointe Church and to refrain from unscriptural conduct in the performance of my services on behalf of the church.

The information contained in the application is correct to the best of my knowledge. I authorize any references or organization listed in the application to give information regarding my character and fitness for serving Oak Pointe Church Ministries. I release all such references from liability for any damage that may result from finishing such evaluations to you and I understand that my omission of material fact on the application may be ground for rejection of the application. I understand that the personal information will be held in confidence by the church staff.

I hereby request and authorize the release of information which pertains to any records or convictions contained in law enforcement files or in any criminal file maintained on me whether local, state, or national. I hereby release local, state, and national law enforcement agencies form any liability resulting from such disclosure.

I further state that I have read the foregoing release and know the contents thereof and sign this release on my own free act.

I hereby authorize the leadership of Oak Pointe Church to make all inquiries regarding my application and to verify all information contained in this application for a volunteer position in Oak Pointe Ministries. I also grant permission for any representative of Oak Pointe Milford to share any information they believe may impact a decision for the volunteer position I am applying for.

I hereby authorize any individuals to disclose all information to Oak Pointe Milford. I release all such persons or entities from liability that may result or arise from Oak Pointe Church's collection of all information for its consideration of my application. This release is in effect ongoing if I am attending Oak Pointe Church Milford.

I declare under penalty of perjury, under the laws of the State of Michigan that the foregoing is true and correct. I hereby request the Michigan State Police and/or Frist Advantage National Criminal Search to release any information which pertains to any record of convictions contained in its file or in any criminal file maintained on me whether local, state, or national.
